Waiver Process

PCCLD has a new Waiver Process for participants in library programs that involve a higher risk for injury or illness. The Maker Committee in 2019 developed three Waiver forms to cover: Maker Activities, Physical Activities and Food Programs; and a permission form for teen lock-ins. The reason behind this effort was to ensure the legal integrity of the documents we use and implement consistent use whenever a heightened risk to the participant is present in programming. These forms can not be modified without the written permission of the Associate Executive Director or Executive Director. Read the attached documents to become familiar with when to use a waiver, how to process waivers, and how to access waiver records. These forms are also available on the portal under Documents /Public Services. Thank you!
