Incident Report Form

Reports should be written as soon as possible after the event. In the case of accidents or injuries, the first priority is for the victim’s immediate aid. Any time someone is sick or gets hurt and bodily fluids need to be cleaned up, the Facilities Superintendent should be notified. The first and primary concern when faced with any emergency is to call 911. It is extremely important that staff DO NOT clean up the area themselves. The contaminated area should be roped off in some manner to prevent others from coming into contact with bodily fluids until Facilities can properly clean and sanitize the area. If the area that is soiled is in a place where it will affect other patrons, clear the library out and close until it is cleaned. The Facilities Department has someone on call at all times who is trained to safely manage bodily fluids. If the Facilities Superintendent cannot be reached, contact another Facilities staff member using the current cell phone numbers.
 
Reminder on How To Write a Good Incident Report (Adapted from http://www.securityguardtraininghq.com/how-to-write-a-detailed-incident-report/)
1. PROVIDE THE FACTS. Do not include statements that reflect judgments or opinion.
2. KEEP IT SIMPLE. Use simple language that most people could understand.
3. FORMAT. Be concise! Long paragraphs are distracting and unnecessary.
4. KEY ELEMENTS. Answer all the questions: who, what, when, where, why and how.
5. MINDFULNESS. Be mindful of the words you chose and avoid language that could be considered biased or discriminatory.
 
Patron does not have to sign suspension paper(s), you can verbally tell the patron(s) that they are suspended.
1) Call security guard and do not approach patron(s) until you have security escort, security guard should be standing between the patron and the PIC and not let the patron get close to the PIC and or staff member.
2) Guard and PIC should be taking picture(s) of the suspended patron(s) the phones have better images than security camera's.
3) Guard should escort suspended patron(s) off the library grounds and remind the suspended patron(s) about not returning to any library branches until they have been reinstated. If guards are not following the procedures or you have concerns please let Security Coordinator know asap.
